The stone is natural, and it is unheated, and in star corundum those are the two questions a buyer most wants settled by a report rather than by eye. Color is the other distinction. Star sapphire is encountered overwhelmingly in blue and gray, and a near colorless white star carrying a legible six ray asterism is a considerably scarcer article.
